Internship application forms (3 copies) can be obtained from the student affairs office or from the announcement page linked below.

After completing the application forms:

a. They must be signed by the workplace where the internship will take place. Then, in order:

b. The supervising faculty member (signs the internship logbook),

c. The head of the department (signs the application form), and

d. The school-industry coordinator (deputy principal) must sign them.
